The Applied Bank Switch Kit provides you step by step instructions for making the change, and provides you all of the necessary tools to convert your direct deposit and automated monthly deductions to your new account.
Here is how it works:
Complete the necessary forms below and call us at 1-888-839-7952 for an appointment.
You’ll need the following things to get started.
Direct Deposits – including any payroll, automated payments, or dividend deposits.
Automatic payments or withdrawals – including recurring bills, club membership dues, charitable contributions, recurring debit card charges, or prescheduled transfers.
Online bill payments – print your existing payee screens to use as a reference when setting up your Applied Bank bill pay service. You will need the payee name, address, phone number, and account number for each of the bills you wish to pay online.
To close your account at your former bank, make sure all of your outstanding checks have cleared your account and you have successfully converted all of your automated direct deposits, automatic payments and withdrawals, and online bill payments. Then, complete the Account Closure Request Form and mail it to your former bank.
